Vaccine Information:

ELIGIBLE INDIVIDUALS 

In accordance with federal and state guidelines, all individuals age 12 and older, who are Florida residents or who are present in Florida for the purpose of providing goods or services for the benefit of residents and visitors, are eligible to receive a COVID-19 vaccine in Seminole County.

At this time, individuals age 12-17 are only eligible to receive the Pfizer vaccine and MUST be accompanied by a parent or guardian when receiving a vaccine. Parents and guardians will need to bring a current identification (with photo and birthdate) to the site.


FLORIDA DEPARTMENT OF HEALTH IN SEMINOLE VACCINE SITES

Pfizer, Moderna and Johnson & Johnson are being administered, depending on availability at Florida Department of Health in Seminole (FDOH-Seminole).

  • LOCATION:
    • Florida Department of Health in Seminole,  400 West Airport Boulevard, Sanford
  • HOURS:
    • Monday-Friday, 9:00a.m. – 3:45p.m. 
  • APPOINTMENTS:
    • Call 407-665-3700 to make an appointment

FDOH-Seminole also offers additional vaccines to ensure safe travels, including: Yellow Fever, Typhoid, Japanese Encephalitis, Cholera. Call 407-665-3700 to schedule an appointment. 


TRUE HEALTH VACCINE SITES

Pfizer, Moderna and Johnson & Johnson are being administered, depending on location and availability at TrueHealth locations in Central Florida. 

  • LOCATIONS:
    • Main Office, 4930 East Lake Mary Boulevard, Sanford 
      • Offering Moderna vaccines
    • Casselberry Office, 1120 State Road 436, Suite 1600, Casselberry
      • Offering Pfizer, Johnson & Johnson
  • HOURS: 
    • Open 8:00a.m.-6:00p.m., Monday-Thursday
    • Open 8:00a.m.-Noon, Friday
  • APPOINTMENTS:
    • Visit the True Health website and select “Vaccine Scheduling”. Choose the date, time, and True Health location for your COVID-19 vaccine.

VACCINES FOR HOMEBOUND INDIVIDUALS

Homebound individuals should call the Citizen’s Information Line at (407) 665-0000 to assess their need for an in-home vaccination visit. The request will be triaged for eligibility, with paramedics scheduling in-home visits. 

Homebound veterans who served in WW2 or the Korean War can put in a request for an in-home visit through the Florida Department of Veterans’ Affairs. 


LOST VACCINE CARDS

Florida Department of Health in Seminole can assist with replacing lost vaccine cards. Individuals may visit the FDOH office in Sanford, 400 West Airport Boulevard, between 8:00a.m.-3:30p.m., Monday-Friday, to request a same-day replacement card.
